Responsibilities:
- Patient Care and Assessments:
- Conduct thorough and accurate patient assessments, including medical history, vital signs, and current health status.
- Assist in the management of chronic conditions and monitor patients' progress over time.
- Administer medications, injections, and vaccinations as prescribed by the family physician.
- Perform nursing procedures, including wound care, dressing changes, and specimen collection, adhering to best practices and infection control protocols.
- Provide patient education on health conditions, treatment plans, medications, and lifestyle modifications to promote optimal health outcomes.
- Assisting the Family Physician:
- Collaborate with the family physician to support patient care and treatment plans.
- Assist during minor medical procedures and examinations, ensuring a safe and comfortable experience for patients.
- Document patient encounters, treatment interventions, and follow-up plans accurately in electronic health record (EHR) systems.
- Patient Support and Communication:
- Act as a liaison between patients and the healthcare team, addressing patient inquiries, concerns, and appointment scheduling.
- Ensure effective communication and coordination of care with other healthcare professionals, both within the clinic and external specialists.
- Clinical Environment Management:
- Maintain a clean, organized, and well-equipped clinical environment, adhering to safety standards and infection control protocols.
- Prepare examination rooms, medical equipment, and supplies to ensure efficiency during patient visits.
- Continuing Professional Development:
- Stay updated with the latest medical advancements, best practices, and relevant regulations through ongoing professional development activities.
Qualifications:
- Current registration as a Practical Nurse (RPN) with the appropriate regulatory body.
- Minimum of 1 years of experience in a primary care setting, with a strong focus on family medicine.
- Proficiency in using electronic health record (EHR) systems and other clinical software.
- Strong interpersonal and communication skills to interact effectively with patients and healthcare team members.
- Ability to work in a fast-paced environment while maintaining attention to detail and accuracy.
- Demonstrated critical thinking and problem-solving abilities to provide effective patient care.
- Commitment to patient advocacy, confidentiality, and patient-centered care.
- CPR certification and any additional relevant certifications are considered assets.
