If you have already filled out the intake form, you have been added to the waiting list and will be called to book a meet-and-greet appointment.
Please do not call us to inquire about your place on the waitlist or when your appointment will be booked. We will contact you by phone and email when we reach your name on the waitlist.
Please be advised that filling out the intake form does not automatically make you a patient of the clinic. As a result, you cannot be seen before your meet-and-greet appointment, and no care or service will be provided before it. This includes refills or urgent appointments. There are no exceptions. If you need care before your meet-and-greet appointment, you will need to visit a walk-in clinic, urgent care center, or the emergency department.
